We want to make our services better for you and to do that we need to know what you think.

If you have something to say this is the place to do it and we’d rather you told us first.  Recommendations, complaints, praise – we’re happy to hear it all.  Most of our patients and families tell us that they’re happy with our services.  However, we want to go that bit further for our community and if you have ideas or suggestions please let us know.

Halton Haven Hospice is committed to continually improving the quality of the services that we provide and we look to those people who have used our services for feedback on their experience whilst at the Hospice to help us in this

We would, therefore, like to ask you to take part in the Hospice’s survey by scanning the QR code below to complete the survey questionnaire electronically. The questionnaire is completed anonymously; you do not provide your name or any other identifying details.

Alternatively, you can ask a member of staff to get you a paper copy of the survey questionnaire from the Hospice Quality Assurance Team and you could take part in the survey anonymously that way
